To get started, create an account using the link provided on the website (Kareo/Tebra). Complete the pre-appointment surveys, provide your demographic information, intake history, pharmacy details, list of medications, and medical and psychiatric history. Ensure you upload your government-issued ID and book an initial virtual assessment appointment. Also, upload a valid credit card (front and back). All forms must be completed 72 hours prior to the appointment.
Please sign in at least 5 minutes before your scheduled appointment. Ensure your video and microphone are working, and have your photo ID available at every encounter, whether virtual or physical.
Each visit includes an assessment, the development of a collaborative care plan, and a summary of the encounter that will be provided to you once the visit note is completed.
All communication should be done through secure messaging in the portal for adequate protection of client information. Patients gain access to the portal once an appointment is booked. Communication can also be done via email.
For administrative inquiries, send a secure message through the portal. Allow 48 to 96 hours for the support staff to respond.
The frequency of visits depends on your condition. For medical patients, visits are scheduled based on acuity. Mental health patients are seen 2 weeks after the initial visit, then monthly. You may schedule an extra appointment if needed for episodic issues.
