  • Once approved patient will receive email confirmation with appointment data with intake form attached which client will complete
  • Client need Complete the pre-appointment surveys, provide your demographic information, intake history, pharmacy details, list of medications, and medical and psychiatric history. Ensure you upload your government-issued ID and book an initial virtual assessment appointment. Also, upload a valid credit card (front and back). All forms must be completed 72 hours prior to the appointment.
  • On day of the appointment client will arrive the appointment in Practice fusion portal waiting room
  • Once the provider is in the client’s chart, the video visit is initiated
  • Once the video visit is started client will be sent the link to join the visit

Please sign in at least 5 minutes before your scheduled appointment. Ensure your video and microphone are working, and have your photo ID available at every encounter, whether virtual or physical.

Each visit includes an assessment, the development of a collaborative care plan, and a summary of the encounter that will be provided to you once the visit note is completed.

All communication should be done through secure messaging in the portal for adequate protection of client information. Patients gain access to the portal once an appointment is booked. Communication can also be done via email.

For administrative inquiries, send a secure message through the portal. Allow 48 to 96 hours for the support staff to respond.

The frequency of visits depends on your condition. For medical patients, visits are scheduled based on acuity. Mental health patients are seen 2 weeks after the initial visit, then monthly. You may schedule an extra appointment if needed for episodic issues.

Before 48 hours of appointment time:
You can reschedule via the portal or request cancellation through secure messaging. A $20 charge applies if the support staff reschedules you.

Within 48 hours of appointment time:
Cancellation within 48 hours falls under the no-refund policy. You can still reschedule, but an additional consultation fee of 50% of the original cost will be charged.

After a thorough assessment and collaborative discussion, medication may be prescribed. Ensure the correct pharmacy is listed in the portal. For mental health patients, monthly visits are required for ongoing prescriptions, while medical patients must arrange for auto-refills with their pharmacy.

Request your records by signing a medical release form and faxing it to 646-666-5758. Allow 7 days for processing. There is a $100 administrative fee for medical record preparation and $50 for diagnosis attestation letters.

We offer comprehensive medical care for various conditions, including hypertension, diabetes, asthma, COPD, obesity, ED, hair loss, skin conditions, chronic pain, and more. We also specialize in primary care and age rejuvenation treatments.

Our mental health services cover ADHD, anxiety, depression, schizoaffective disorder, schizophrenia, PTSD, and OCD.

We currently do not offer therapy but provide non-pharmacological coping templates for patients with ADHD, anxiety, and depression. We also offer holistic, integrative care that treats the mind, body, and soul. In the future, we plan to incorporate other novel tools that will revolutionize client’s clinical outcome, so stay tuned.

We accept credit cards and cash for office visits. Payments must be made before services are rendered.

  • Initial Psychiatric Evaluation: $240
  • Mental Health Medication Management: $99
  • Initial Medical Patient Intake: $140
  • Follow-up Medical Patient: $89

Other services, such as extended phone calls, report writing, and legal matters, are billed at $100/hour (in 15-minute increments) or $400/hour for legal matters.

If you have a life-threatening emergency, go to the nearest emergency room. If it is not life-threatening but urgent, schedule an appointment, or consult via secure messaging or email.

Yes, patients receiving mental health services should maintain a primary care physician (PCP) to monitor treatment response and handle necessary tests, such as bloodwork and EKGs.

We currently provide services in New York, New Jersey, Arizona, and Washington State, with plans to expand to other states soon.

Office visits depend on the type of treatment you are receiving. If required, you will be notified in advance, and we will schedule a convenient time with you.
